When it is time to leave the Inn, two years of continued support through the After Care Program is offered to each family. Aftercare provides a continuity of care through case management, advocacy, referrals-including transitional and permanent housing, and material supports. Aftercare case managers work with families by visiting them in their homes or meeting with them at one of the three Aftercare locations. The Aftercare case managers also coordinate an annual Christmas gift program, helping sixty families each year.
